Medical Plan
- Coinsurance: Percentage you pay for covered healthcare services after you've met your deductible
- Copay: Set amount you pay for provider visits, prescriptions, and other healthcare services
- Deductible: Amount you pay before insurance starts contributing (except where copays apply)
- Explanation of Benefits (EOB): Explanation of the costs of your claim, how much the plan paid, and any remaining balance
- High-Deductible Health Plan (HDHP): Health plan with a high deductible where you pay the full cost of non-preventive medical care and prescription drugs until you meet the deductible
- Network: Group of providers that the plan contracts with to give you a discounted rate
- Out-of-Network: Providers who aren’t contracted with the plan and usually charge more
- Out-of-Pocket Maximum: Most you'll pay out-of-pocket each year for covered expenses
- Plan Year: Period when your coverage is active (Archrock's is Jan 1–Dec 31.)
- Preferred Provider Organization (PPO): Medical plan that provides coverage through a network of selected healthcare providers
- Premium: Amount paid for your insurance each month; Archrock pays the larger portion of the cost, and you pay the rest
- Preventive Care: Routine healthcare services like check-ups, immunizations, and screenings
- Primary Care Physician (PCP): Provider who is your first point of contact for medical care
- Specialist: Provider who focuses on a specific area of medicine or a group of patients to diagnose, manage, prevent, or treat certain types of symptoms and conditions
Prescription Coverage
- Formulary (Prescription Drug List): List of generic and brand-name prescription medications covered by your health insurance plan
- Generic Drugs: Non-brand drugs with the same active ingredients as, and that typically cost less than, their brand-name counterparts.
Dental Plan
- Calendar Year Maximum: Amount the plan pays per member, per year
Life Insurance Coverage
- Evidence of Insurability (EOI): Application process where you provide health information to be considered for certain types of insurance coverage
- Guaranteed Issue: Amount up to which an EOI isn't needed to be approved for coverage
